Employee Wellness Program: April 1, 2010 - February 4, 2011

Lifestyle Improvement For Employees

Holland Hospital and WebMD are pleased to continue our partnership to provide the LIFE Rewards Employee Wellness Program. Our program empowers and encourages employees and their families to take responsibility for healthy lifestyle choices. We welcome and encourage all employees and spouses to participate in the voluntary LIFE Rewards Program. The program consists of two separate components – employees have the choice to complete one or both.


Component One - Health Premium

This component is for employees and spouses on a Holland Hospital health plan.

Those that are on a Holland Hospital health plan will receive Level 1 health plan premiums in the 2011/2012 fiscal year (rates to be determined) by completing these three easy steps by February 4, 2011:

  1. Complete the WebMD Health Quotient
  2. Complete a Wellness Lab DrawPDF
  3. Attend a Biometric ScreeningPDF

Each step is worth 10 points for a total of 30 points.

Spouses covered on your Holland Hospital health plan will need to complete the three steps.

Results and data from LIFE Rewards will not impact your eligibility for Level 1 health plan premiums.

Employees who choose not to participate in LIFE Rewards will receive Level 2 health plan premiums (rates to be determined).

Upon completion of the three easy steps, you will receive a congratulatory email from WebMD. Please keep this email for your records.

Component Two - Gift Card

This component is for all employees. Spouses are ineligible for the $20 gift card.

All employees will have the opportunity to earn additional wellness incentives throughout the year – including a $20 gift card.

In order to receive your $20 Meijer Gift Card, you must complete the following two items and any of the “Healthy Action” items for
a total of 20 points by February 4, 2011.

  1. No Tobacco Use: Self-reported on WebMD under your “Rewards” section.
  2. Complete Health Quotient: Login to WebMD and complete your Health Quotient.

Pick and choose your favorite "Healthy Actions." Each action is worth 5 points. Some examples are:

    • Complete the WebMD Nutrition Improvement program.
    • Complete your annual physical/recommended care.
    • Exercise at the Center for Good Health or Cardiac Rehab.

What if my spouse and I are not on a Holland Hospital health plan?

If you and your spouse are not on a Holland Hospital health plan, you and your spouse are still encouraged to participate. Please note, spouses not on a Holland Hospital health plan need to fill out the Spouse EnrollmentPDF form.

There are plenty of great reasons to participate.

If you and your spouse decide to participate, please remember that all your health information is completely confidential and secure. Our partnership with WebMD offers a private website to help track your health progress.

The LIFE Rewards Employee Wellness Program is easy, free and completely confidential. Feel better, get active and save money while improving your health. It is in everyone’s best interest for a healthier future!
